If you lost your savings bonds, you can fill out the form PD F 1048 shown above and send it to the following address. You can replace the lost savings bonds even if you do not know the serials numbers of the savings bonds and do not remember much information about it. The Treasury Department has a record of all US savings bonds that can be searched whenever you fill out the form above and sent to them.
